What this assessment is
Percocet is an opioid prescribed for short-term pain, and it is also highly addictive. These questions are about how you are using it and whether that use has started to cause problems.
It combines oxycodone, a strong opioid pain reliever, with acetaminophen, and it is usually prescribed after surgery or an injury rather than for the long term.
The questions are confidential and take a few minutes: whether you have taken more than you were prescribed, whether you have needed more for the same effect, and whether you have tried to stop. This is a starting point for reflection, not a medical diagnosis, and a licensed clinician can carry out a full assessment if the answers suggest one is needed.
